Role Description

This full-time, on-site Licensed Practical Nurse (LPN) position is located in Houston, TX. The LPN will be responsible for providing patient care under the supervision of a registered nurse or physician, administering medications, monitoring vital signs, and maintaining patient records. Additional responsibilities include assisting with treatments, educating patients and families on care plans, and ensuring a safe and comfortable environment for all patients. The LPN will collaborate with the broader healthcare team to ensure high-quality care delivery.

Required Documentation

 ☐ Current, updated resume/CV

 ☐ Active, unencumbered professional license(s) and/or certifications

 ☐ Two (2) professional references

 ☐ Emergency contact information

Health & Immunization Requirements

 ☐ Hepatitis B vaccination records or signed declination/exemption form

 ☐ Current BLS/CPR certification

 ☐ TB screening results (PPD/IGRA) or chest X-ray documentation

Competency & Screening Requirements

 ☐ Completed hospice skills checklist and required online competency assessment

 ☐ Satisfactory background check results

 ☐ Drug screening results

Identification & Employment Verification

 ☐ Valid driver’s license

 ☐ Social Security Number (SSN) or approved employment verification documentation
