Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management for pain control, hospice care, or chronic conditions.
- Assist with diagnostic evaluations and procedures such as catheterization, specimen collection/processing, and wound care using aseptic techniques.
- Manage airway and ventilator support for patients requiring respiratory assistance; provide emergency first aid and participate in airway management during acute episodes.
- Document all patient interactions thoroughly in medical records while maintaining strict adherence to HIPAA regulations and clinical confidentiality policies.
- Collaborate with interdisciplinary teams to develop discharge plans, coordinate case management efforts, and ensure seamless continuity of care for patients with complex needs like dementia or developmental disabilities.
- Provide education to patients and families on health coaching, medication administration, nutrition, and long-term care regulations to promote informed decision-making.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice.
- Proven experience working with diverse populations including geriatrics, pediatrics, neonatal or adult patients in settings such as long-term care facilities, hospitals (including ICU or trauma centers), outpatient clinics, or home care environments.
- Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/10), and clinical procedures such as venipuncture, tube feeding, catheter care, and basic life support (BLS).
- Experience with EMR/EHR systems like Epic, Cerner or Athenahealth; familiarity with medical imaging documentation is a plus.
- Critical care experience (ICU or Level I/II trauma centers), emergency medicine exposure, or hospice experience highly preferred.
- Ability to perform physical examinations accurately and conduct diagnostic evaluations efficiently while demonstrating excellent caregiver communication skills.
- Knowledge of infection control protocols and long-term care regulations; experience working with individuals with disabilities or dementia is advantageous.
