Position Overview:
The RN Case Supervisor plays a critical leadership role, overseeing the provision of skilled nursing care and supporting interdisciplinary teams to ensure optimal patient outcomes. This role requires clinical expertise, operational efficiency, leadership, and a deep commitment to patient-centered care aligned with Prime Home Health’s mission and values.
You will report directly to the Regional Clinical Director and work closely with operations, nursing field staff (LVNs, HHAs), and client families to deliver excellence.
Key Responsibilities:
Clinical Oversight
- Conduct initial home health visits and regular re-evaluations. Monitor patient conditions, treatments, and medications. Notify physicians of changes in patient status.
Care Coordination
- Coordinate and monitor all aspects of patient care. Supervise LVNs, HHAs, and other field staff. Participate in patient case conferences and interdisciplinary team meetings.
Compliance & Documentation
- Complete all required OASIS documentation and clinical notes timely and accurately. Ensure compliance with Medicare, Medicaid, State, and Agency policies.
Patient & Family Education
- Educate patients and families on disease processes, medications, and care plans. Advocate for patient needs and foster self-management techniques.
Leadership & Mentorship
- Provide mentorship, education, and leadership to field nursing staff. Foster a culture of compassion, professionalism, and clinical excellence.
Quality Assurance
- Participate in Quality Improvement initiatives. Identify trends and support action plans to enhance patient care and staff development.
Qualifications:
- Active Registered Nurse (RN) license in California.
- Minimum of two (2) years of recent experience in a Home Health Agency, primary care clinic, or related health facility.
- Clinical knowledge in managing a diverse patient population (subacute, geriatric, chronic illness, post-acute recovery).
- Proficiency with OASIS documentation preferred.
- Strong leadership, communication, organizational, and critical-thinking skills.
- Ability to work independently while thriving in a collaborative team environment.

Work Environment & Physical Requirements:
- Ability to travel locally to patient homes across assigned regions.
- Ability to lift and maneuver patients or equipment if necessary.
- Regular exposure to various home environments and conditions.
- Frequent use of computers, telehealth platforms, and mobile applications.
