Responsibilities
- Conduct comprehensive patient evaluations to determine physical therapy needs, including assessing physiology, anatomy, and medical history.
- Develop and implement tailored treatment plans that incorporate evidence-based practices such as trigger point therapy, orthopedics, neurology, pediatrics and geriatrics.
- Manage patient care through active treatment sessions focused on rehabilitation, mobility enhancement, pain management, and functional improvement.
- Collaborate closely with physicians, nurses, occupational therapists, and other healthcare providers to coordinate plan of care and discharge strategies.
- Maintain accurate clinical documentation in accordance with healthcare standards and electronic health records (EHR) or electronic medical records (EMR) systems.
- Engage in discharge planning by preparing detailed reports on patient progress and recommending follow-up care or community resources.
- Educate patients and their families on treatment procedures, home exercises, safety protocols, and community resources to promote ongoing recovery outside clinical settings.
Qualifications
- Valid New Jersey Physical Therapist license.
- Proven experience practicing in outpatient clinics.
- Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques including trigger point therapy.
- Experience working with geriatric patients, pediatrics, neurology cases, orthopedics, and outpatient rehabilitation centers is highly desirable.
- Familiarity with clinical documentation standards and EMR/EHR systems for accurate patient record management.
- Excellent client communication skills with the ability to educate patients effectively about their treatment plans.
- Ability to work collaboratively within multidisciplinary teams to develop comprehensive care plans and ensure optimal patient outcomes.

Pay: $110,000.00 - $130,000.00 per year
