Pride Health is hiring an experienced RN Home Health for a 13-week contract position in Florence, AL.

Position: RN Home Health
Location: Florence, AL

Travel pay: $2,186/week

Schedule: 5x8 Days (40 hours/week)
Contract Duration: 13 weeks
Start Date: ASAP

Coverage Area: Lauderdale and Colbert Counties
On call: On-call every 8 weeks

Weekly Productivity:
30 units per week

Job Description

The Registered Nurse (RN) will provide skilled nursing care to home health patients of all ages in their place of residence. This role involves coordinating care with the interdisciplinary team, patients/families, and referring agencies, while assuming responsibility for overall care coordination.

Key Responsibilities

• Assess home health patients in stable and emergent situations
• Develop, implement, document, and evaluate individualized Plans of Care (POC)
• Ensure continuity and quality of care with accurate, timely documentation
• Educate patients and families on care plans, medications, diet, and home safety
• Supervise and provide clinical direction to HHAs/LVNs/STNAs
• Perform wound care, IV care, venipuncture, catheter and drain care
• Monitor pain management and educate patients/families accordingly
• Arrange referrals and ensure payer compliance
• Participate in interdisciplinary team meetings
• Perform on-call duties as scheduled

Experience Required

• 2 years of Home Health experience
• 1–2 years of RN experience in a clinical care setting
• OASIS experience
• Homecare Homebase (HCHB) software experience

Skills & Certifications Required

• Current RN license
• BLS/CPR certification
• Valid driver’s license
• Reliable transportation and active auto insurance

Preferred:

• BSN

Education

• Associate Degree in Nursing or Nursing Diploma – Required
• Bachelor of Science in Nursing (BSN) – Preferred
