- The primary responsibility is to promote the growth, well-being and autonomy of the individuals served. Growth refers to the maturation of the individual in an age-appropriate manner. Well-being requires that the Program Coordinator (PC) use programs and techniques which reflect the preferences of the individual. Autonomy is the ability of the individual to have control over their environments and life events. The PC is to assist the individual in defining what autonomy means to them through the self assessment process and to ensure that they have the support needed to maintain control over their environments and life events.
- Complete a Self Assessment annually with each individual to determine personal outcomes for this individual.
- Facilitate all plan meetings to discuss a strategy in implementing the personal outcomes as stated in the individual=s self assessment. As a facilitator, the PC is to ensure that the least restrictive, most appropriate plan is in place to meet the needs and desires of the individual requesting services.
- Complete documentation requirements to include progress on the plan, goals and desires of the person, plan meeting/review minutes, Human Rights and Behavior Support Committee minutes, Implementation Strategies, etc. Assess need for changes when/if the individual expresses dissatisfaction and ensure the plan is modified accordingly.
- Assess data on programs/guidelines to ensure the individual is learning the skill or behavior that is more adaptive and socially productive.
- Act as a liaison to family members/guardians, team members, and outside entities to ensure continuity and satisfaction of services is a priority.
- Complete required monthly site visits.
- Coordinate and assist individual in changes in services and environments: (move to a new apartment, transfer to a new setting, etc.).
- Attend agency staff meetings, participate in agency sponsored in service training, and enhance professional development by reading, and attending job related workshops and classes.
VIII. POSITION RELATED QUALIFICATIONS
EDUCATION
- Bachelor’s degree in Social Work, Social/Behavioral Sciences, Psychology, Education or related fields.
- At least 1 year of experience working with people with developmental disabilities.
- At least one year of experience in working with the Personal Outcome Measures established by The Council is preferred.
- Assistance with transferring and lifting up to 50 pounds.
