Travel Physical Therapist

Pride Health is hiring a Travel Physical Therapist, Home Health/Hospice to support our client’s medical facility in Ontario, OH 44906. This is a 3 month contract assignment (convertible to permanent based on your work performance). and a great way to start working with a top-tier healthcare organization!

Specific Duties:

• Evaluate patients’ functional status and physical therapy needs in compliance with CMS guidelines.
• Develop and implement individualized treatment plans in collaboration with physicians.
• Deliver evidence-based therapy to relieve pain, restore function, and improve mobility.
• Monitor and document patient progress, reporting changes to physicians and nurse supervisors.
• Educate patients, families, and caregivers on treatment plans, progress, and functional limitations.

Requirements:

• Education: Experience in home health or related clinical settings preferred.
• License(s): Active Physical Therapist license
• Certification(s): Current CPR certification.
• Years of experience: 1 year
• Skills/Specialties/Technology: Home Health

• Location: Ontario, OH 44906
• Job Type: 3 months contract to hire
• Schedule: Monday through Friday
• Shift start & end times: Monday-Friday, 8:00 am-4:30 pm
• Workdays per week: 5 days
• Pay Range: $50-56/hrs. on W2 without benefits
