What are the responsibilities and job description for the Transplant Nurse position at Pride Health?
We are seeking a dedicated Registered Nurse (RN) to support a transplant program in an outpatient/clinic setting. This role focuses on triage coordination, laboratory follow-ups, and patient care management in collaboration with physicians and nurse practitioners. The ideal candidate is highly organized, communicative, and experienced in managing complex patient care workflows.
Key Responsibilities
- Triage patient phone calls and communicate needs to the care team (NP/MD)
- Review outpatient laboratory results in collaboration with providers
- Communicate medication changes and care plan updates to patients and caregivers
- Provide support during clinic visits and follow-up care
- Perform medication reconciliation and review of systems
- Coordinate follow-ups, including lab reviews and medication adjustments
- Educate patients and families on health conditions and treatment plans
- Assist with administrative duties such as prior authorizations, forms, and documentation
- Manage communication tasks including faxes and care coordination updates
- Collaborate with interdisciplinary teams to ensure continuity of care
Qualifications
- Active Registered Nurse (RN) license (state-specific requirement)
- BLS and PALS certifications (AHA preferred/required)
- Previous 2 years RN experience required; transplant or outpatient experience required
- Experience with EPIC or similar electronic medical records system preferred
- Strong clinical assessment, communication, and organizational skills
- Ability to work effectively in a fast-paced, team-based environment
- Commitment to high-quality patient care and education
Important Details:
- 13-week contract position
- Shift is 12 hrs/day (7 AM - 7:30 PM), 36 hrs/week
- Location is New York City, NY 10032
- Pay Range for Locals: $65 - $70 per hour
