Transplant Nurse

We are seeking a dedicated Registered Nurse (RN) to support a transplant program in an outpatient/clinic setting. This role focuses on triage coordination, laboratory follow-ups, and patient care management in collaboration with physicians and nurse practitioners. The ideal candidate is highly organized, communicative, and experienced in managing complex patient care workflows.

Key Responsibilities

• Triage patient phone calls and communicate needs to the care team (NP/MD)
• Review outpatient laboratory results in collaboration with providers
• Communicate medication changes and care plan updates to patients and caregivers
• Provide support during clinic visits and follow-up care
• Perform medication reconciliation and review of systems
• Coordinate follow-ups, including lab reviews and medication adjustments
• Educate patients and families on health conditions and treatment plans
• Assist with administrative duties such as prior authorizations, forms, and documentation
• Manage communication tasks including faxes and care coordination updates
• Collaborate with interdisciplinary teams to ensure continuity of care

Qualifications

• Active Registered Nurse (RN) license (state-specific requirement)
• BLS and PALS certifications (AHA preferred/required)
• Previous 2 years RN experience required; transplant or outpatient experience required
• Experience with EPIC or similar electronic medical records system preferred
• Strong clinical assessment, communication, and organizational skills
• Ability to work effectively in a fast-paced, team-based environment
• Commitment to high-quality patient care and education

Important Details:

• 13-week contract position
• Shift is 12 hrs/day (7 AM - 7:30 PM), 36 hrs/week
• Location is New York City, NY 10032
• Pay Range for Locals: $65 - $70 per hour
