RN Intake Outreach Manager – Certified Home Health Agency

We are seeking a compassionate and detail-oriented RN Intake Outreach Manager to oversee the referral, screening, and admission process for our Certified Home Health Agency. This role ensures the safe and timely transfer of patients from hospitals and community settings into home health services, while also developing new referral sources.
Key Responsibilities
- Manage the intake, evaluation, and admission process from inquiry to acceptance.
- Coordinate scheduling of Nurse Care Managers and vendors for timely evaluations.
- Serve as a clinical resource for referral partners and staff.
- Collaborate with leadership to expand referral networks.
- Perform on-call duties and other assigned responsibilities.
Qualifications
- Current NYS RN license; graduate of an accredited nursing program.
- 2 years of experience in hospital discharge planning or home care admissions.
- Strong multitasking and prioritization skills.
- Excellent communication (verbal/written) and organizational abilities.
- Knowledge of home care regulations and practices.
- Valid driver’s license: MS Office proficiency required.
Compensation & Benefits
- Salary: $105,000 – $110,000 annually
