Position Overview:
Seeking a dedicated Registered Nurse to provide high quality home health care and case management services. This role focuses on delivering skilled nursing care while coordinating patient care plans to ensure optimal outcomes, safety, and independence in the home setting.
Schedule & Work Details:
- Location: Lucas, Wood, Fulton, Hancock, Henry, and surrounding counties
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hours: 40 hours, with a 36 hour guarantee
- On Call/Weekends: 1 week and 1 weekend per month
- Productivity Expectation: 30 points
- Pay Rate: $51 to $53 per hour
- Mileage Reimbursement: Yes, reimbursed between patient visits
Key Responsibilities:
- Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans
- Manage a caseload of patients, including care coordination, planning, and ongoing case management
- Conduct comprehensive assessments and reassessments to evaluate patient needs and progress
- Educate patients and families on diagnoses, treatment plans, medications, and safety practices
- Administer treatments, medications, and interventions safely and accurately
- Maintain thorough and timely documentation using electronic medical record systems
- Collaborate with physicians, therapists, and interdisciplinary care teams to ensure continuity of care
- Monitor patient outcomes and adjust care plans to improve quality and effectiveness
Required Qualifications:
- Active Registered Nurse (RN) license in the state of Ohio
- Recent home health experience required
- Strong case management and organizational skills
- Ability to work independently and manage a patient caseload effectively
- Valid driver’s license and reliable transportation
Preferred Qualifications:
- Experience with Homecare Homebase (HCHB)
- Strong communication and patient education skills
- Familiarity with home health regulatory standards and best practices
