Registered Nurse (Home Health)

Pride Health is hiring a Registered Nurse - Home Health to support our client's healthcare facility in Lexington, KY.

Schedule: Monday-Friday | 8:00 AM-4:30 PM
Location: Lexington, KY
Coverage Area: Fayette, Jessamine, Garrard, Madison, Powell, Montgomery, and Clark County
Job Type: Local contract
Contract Length: 18 Weeks (Possibility to Extend)
Pay Rate: $53.00 - $56.00/hour (W-2) Plus Mileage reimbursement
Mileage Reimbursement Rate: $0.51/ mile
*Pay offered is based on experience, expertise, credentialing, and education.

Duties: 

• Provide skilled nursing care to patients in the home setting, performing comprehensive assessments, developing individualized care plans, and monitoring patient progress in accordance with physician orders.
• Educate patients and caregivers on disease management, medication administration, infection prevention, medical equipment uses, and self-care techniques to improve health outcomes and promote independence.
• Maintain accurate clinical documentation, communicate changes in patient condition to the interdisciplinary healthcare team, and ensure compliance with agency policies, Medicare guidelines, and professional nursing standards.

 
Requirements: 

• License(s): Current and unrestricted RN licensure in state of practice
• Certification(s): Current CPR Certification
• Years of experience: 1 years of Home Health experience and Homecare Homebase (HCHB) is a plus
