Position Overview:
Seeking an experienced Registered Nurse to provide high-quality, patient-centered care in a home health setting. This role involves delivering skilled nursing services to patients across multiple counties while ensuring compliance with clinical standards and documentation requirements. The ideal candidate has prior home health experience and is proficient with Homecare Homebase (HCHB).
Schedule & Work Details:
Location: Huntington, WV covering Putnam, Cabell, and Wayne counties
Schedule: Monday through Friday, 8:00 AM to 4:30 PM
On Call: One full week every 4 to 6 weeks
Weekend Requirement: Included as part of on-call rotation
Mileage Reimbursement: Yes, reimbursed between patient visits from first to last visit
Weekly Hours: Full-time schedule
Pay Rate: $51 to $53 per hour
Key Responsibilities:
- Provide skilled nursing care to home health patients based on physician orders and care plans
- Complete point-of-care visits on a per-visit basis (PPV)
- Perform comprehensive assessments and ongoing evaluations of patient needs
- Administer medications, treatments, and interventions as required
- Accurately document all patient care in Homecare Homebase (HCHB)
- Educate patients and families on disease management, medications, and care plans
- Collaborate with interdisciplinary team members to ensure continuity of care
- Participate in on-call rotation and respond to patient needs as required
Required Qualifications:
- Active Registered Nurse (RN) license in the state of West Virginia
- Minimum of 1 year of home health experience
- Experience using Homecare Homebase (HCHB)
- Strong clinical assessment and documentation skills
- Valid driver’s license and reliable transportation
Preferred Qualifications:
- Additional experience in home health settings
- Strong familiarity with HCHB workflows and documentation
- Excellent communication and patient education skills
- Ability to work independently and manage time effectively
