Position Overview
We are seeking a compassionate and skilled Registered Nurse to deliver high-quality, patient-centered care in a home health setting. This role focuses on providing comprehensive nursing services, coordinating care with interdisciplinary teams, and ensuring positive patient outcomes through accurate assessments and individualized care planning.

Schedule & Work Details
Shift: Monday through Friday, 8:00 AM to 4:30 PM
On-Call/Weekend Requirements: On-call and weekends as scheduled
Coverage Area: Assigned service area based on patient needs
Mileage Reimbursement: Yes, reimbursed between patient visits from first to last patient
Pay Rate: $51 to $53 per hour

Key Responsibilities

  • Provide skilled nursing care to patients in their homes, ensuring compliance with care plans and physician orders
  • Perform comprehensive patient assessments and develop individualized plans of care
  • Monitor patient conditions, evaluate progress, and adjust care plans as needed
  • Administer medications and treatments in accordance with established protocols
  • Educate patients and families on disease management, medications, and post-care instructions
  • Collaborate with physicians, therapists, and other healthcare professionals to coordinate care
  • Maintain accurate and timely documentation in accordance with agency and regulatory standards
  • Participate in on-call rotation and provide care as needed during scheduled weekends

Required Qualifications

  • Active Registered Nurse (RN) license
  • Strong clinical assessment and patient care skills
  • Ability to work independently in a field-based environment
  • Reliable transportation and valid driver’s license

Preferred Qualifications

  • Prior home health experience
  • Strong communication and organizational skills
  • Experience with electronic documentation systems
  • Ability to manage time effectively and maintain productivity
