Position Overview
We are seeking an experienced Home Health Registered Nurse to provide high quality, patient-centered care in the Olympia area and surrounding communities. This position focuses on delivering skilled nursing services in patients’ homes while ensuring compliance with clinical standards and productivity expectations.

Schedule & Work Details

  • Location: Olympia, WA and surrounding areas
  • Shift: Day shift
  • Weekly Hour Guarantee: 32 hours
  • Productivity Expectation: 30 points
  • Pay Rate: $51 to $53 per hour
  • On Call/Weekend Requirement: Not required
  • Mileage Reimbursement: Yes, reimbursed between patient visits only, not from home to first patient or last patient to home

Key Responsibilities

  • Provide skilled nursing care to home health patients in accordance with physician orders and care plans
  • Perform patient assessments, including physical, psychosocial, and environmental evaluations
  • Develop, implement, and update individualized care plans
  • Administer medications and treatments as prescribed
  • Monitor patient progress and adjust care plans as needed
  • Accurately document all visits, assessments, and care provided using Homecare Homebase (HCHB)
  • Educate patients and families on disease management, medications, and post-care instructions
  • Coordinate care with physicians, therapists, and interdisciplinary team members
  • Ensure compliance with all regulatory and agency standards

Required Qualifications

  • Active Registered Nurse (RN) license
  • Minimum of one year Home Health experience
  • Hands-on experience with Homecare Homebase (HCHB)
  • Strong clinical assessment and documentation skills
  • Ability to work independently and manage a flexible schedule

Preferred Qualifications

  • Prior experience in a fast-paced home health environment
  • Strong communication and patient education skills
  • Excellent time management and organizational abilities
  • Ability to adapt to varying patient needs and home environments
