Title: Nurse Practitioner
Location: Fayetteville, NC
Duration: 13 weeks
Shift: Monday–Friday, 8:00 AM – 5:00 PM
Schedule:40 hours
Hourly Pay Rate: $66-$70
Must-Have Qualifications & Experience:
Education:
NP: Master’s or Doctorate in Nursing (MSN/DNP) from an accredited program.
PA: Degree from an accredited Physician Assistant program.
Licensure & Certification:
Active and unrestricted NP or PA license (state of practice).
NP certified by AANP or ANCC in FNP, ANP, GNP, or AGACNP.
PA certified by NCCPA.
Active DEA license and prescriptive authority.
Fully COVID-19 vaccinated (proof required).
Experience:
Minimum 1 year of clinical experience (preferably in long-term care, geriatrics, or primary care).
Skilled in managing acute and chronic conditions and performing comprehensive assessments.
Experience with advanced care planning and interdisciplinary team collaboration.
Proficient in EMR documentation and digital health tools.
Roles & Responsibilities
Conduct comprehensive medical assessments for high-risk members in facility settings.
Diagnose and manage acute and chronic medical as well as behavioral health conditions.
Document accurately and completely using ICD-10 coding standards.
Collaborate with an interdisciplinary care team including physicians, nurses, social workers, and therapists.
Lead advanced illness and advanced care planning discussions with patients and families.
Provide counseling and education to patients and caregivers regarding treatment plans, health promotion, and disease prevention.
Coordinate care transitions and follow-up to ensure continuity and quality of care.
Maintain compliance with clinical protocols, documentation standards, and regulatory requirements.
Participate in team meetings, case reviews, and quality improvement initiatives.
Travel between 2–3 senior living facilities in the Fayetteville, NC area during shifts.
