Pride Health is hiring a Nurse Practitioner for our client in NC! (Contract to hire)
Location: Wake & Chatham Counties, North Carolina (travel between multiple LTC facilities)
Schedule: Monday-Friday, 8 AM-5 PM
Duration: 1 year contract to hire
Pay: 65-70/hr
Primary Responsibilities
- Conduct comprehensive clinical assessments for members residing in assigned facilities
- Independently manage acute and chronic medical and behavioral health conditions
- Ensure high‑quality, complete documentation including ICD‑10 coding
- Communicate and collaborate with interdisciplinary care teams, including physicians, nursing staff, and facility leadership
- Lead advanced illness and advanced care planning conversations
- Provide education, counseling, and support to patients, caregivers, and facility staff
- Participate in team-based meetings and case discussions as needed
- Support continuity and coordination of care across multiple settings
Required Qualifications
- Active and unrestricted NP or PA license in state of residence (or ability to obtain by start date)
Education Requirements:
Nurse Practitioner:
- Graduate of an accredited MSN or DNP program
- Board certification through AANP or ANCC in one of the following:
- Family Nurse Practitioner (FNP)
- Adult Nurse Practitioner
- Gerontology Nurse Practitioner
- Adult-Gerontology Acute Care Nurse Practitioner
Physician Assistant:
- Graduate of an accredited PA program
- Board certified through NCCPA
Additional Requirements:
- Current and active DEA licensure/prescriptive authority
- Reliable transportation for travel between facilities
Preferred Qualifications
- 1 year of clinical experience (long‑term care strongly preferred)
- Experience with patients who have complex behavioral, social, or functional needs
- Strong understanding of geriatrics, chronic illness, and end‑of‑life care
- Experience conducting advanced illness discussions
- Proficiency with EMR systems and electronic documentation
- Excellent communication, time management, and interdisciplinary collaboration skills
