We are seeking a dedicated and compassionate Physical Therapist (PT) to join our Home Health team serving patients throughout Reno, Sparks, Carson City, Minden, and Gardnerville. The Physical Therapist will provide high-quality, patient-centered rehabilitative care in the home setting, helping patients restore mobility, improve functional independence, and enhance overall quality of life.

• Evaluate patients and develop individualized physical therapy treatment plans based on physician orders and clinical findings.
• Provide skilled physical therapy interventions to improve strength, mobility, balance, endurance, and functional independence.
• Conduct comprehensive assessments and regularly evaluate patient progress toward established goals.
• Educate patients, families, and caregivers on therapeutic exercises, safety techniques, fall prevention, and home exercise programs.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to ensure coordinated patient care.
• Accurately document evaluations, treatments, patient responses, and progress according to agency, state, and federal guidelines.
• Maintain productivity standards while delivering exceptional patient care and clinical outcomes.
• Promote patient independence and help prevent hospitalizations through effective rehabilitation strategies.

• Active State of Nevada Physical Therapist (PT) License.
• Minimum 1 year of Home Health experience.
• Graduate of an accredited Physical Therapy program.
• Strong assessment, treatment planning, and patient education skills.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently while managing an assigned territory and caseload.
