Position Overview:
Seeking a Licensed Practical Nurse (LPN) to provide high-quality patient care in a home health setting. This role focuses on delivering nursing services in accordance with established care plans while collaborating with the healthcare team to ensure optimal patient outcomes.
Schedule & Work Details:
- Pay Rate: $28 to $30 per hour
- Weekly Hour Guarantee: 36 hours per week unless PTO is taken; minimum hours guaranteed if closed for holidays
- Travel: Required between patient visits
- Mileage Reimbursement: Yes, between patient visits
- On Call/Weekend Requirement: 1 weekend call per month or PRN
Key Responsibilities:
- Provide nursing care within the scope of practice as defined by state laws
- Follow the established plan of care and collaborate with the healthcare team
- Observe, record, and report patient conditions and responses to treatment daily to the supervising nurse or physician
- Educate patients and their families on care plans and maintaining a safe and healthy home environment
- Maintain accurate and timely documentation of all patient interactions and care provided
Required Qualifications:
- Current LPN licensure in the state of practice
- Current CPR certification
- Valid driver’s license, active vehicle insurance, and access to reliable transportation or public transportation
Preferred Qualifications:
- Previous home health or travel nursing experience
- Strong communication and patient education skills
- Ability to work independently and manage time effectively
