We are seeking a compassionate and experienced Social Worker (LCSW) to provide support to patients and families in a hospital-based setting. In this role, you will assess psychosocial needs, develop care plans, and connect individuals with appropriate community and healthcare resources. Key Responsibilities
- Conduct comprehensive psychosocial assessments for patients and families
- Develop and implement individualized care and discharge plans
- Collaborate with interdisciplinary healthcare teams to ensure continuity of care
- Provide crisis intervention and emotional support as needed
- Connect patients with community resources, support services, and follow-up care
- Advocate for patient needs and ensure access to appropriate services
- Maintain accurate and timely documentation in compliance with regulations
Qualifications
- Active Licensed Clinical Social Worker (LCSW) certification required
- Minimum 2 years of experience in a hospital or acute care setting
- Prior experience in acute care hospitals is required
- AHA BLS certification required
- Strong communication, assessment, and case management skills
- Ability to work effectively in a fast-paced, team-oriented environment
Important Details:
- 14-week contract position
- Shift is 12 hrs/day shift (8:00 AM-8:30 PM), 36 hrs/week
- Location is Canton, GA 30115
- Locals are not accepted within 50 miles of the facility.
- Candidate will be considered as per the driving license address
- Pay Range for travelers: $1,700 - $1,740 per week
