Position Overview
Seeking a dedicated Registered Nurse (RN) to provide high-quality home health services to patients in the Dothan area and surrounding communities. This role is responsible for delivering skilled nursing care, developing and managing individualized care plans, and collaborating with physicians and interdisciplinary team members to ensure optimal patient outcomes.
Schedule & Work Details
- Location: Dothan, AL and surrounding areas
- Schedule: Monday through Friday, 8:00 AM to 4:30 PM
- Weekly Hour Guarantee: 40 hours
- On-Call Requirement: 1 weekend every 4 weeks and 1 to 2 call nights per week, including triaging and patient visits
- Mileage Reimbursement: Yes, between patient visits (first to last patient)
- Pay Rate: $51 to $53 per hour
Key Responsibilities
Clinical Competence
- Initiate, develop, implement, and revise individualized plans of care in collaboration with physicians and healthcare team members
- Supervise and support home health aides and licensed practical nurses, ensuring compliance with state and federal regulations
- Provide required supervisory visits and clinical oversight
Documentation and Care Delivery
- Deliver high-quality clinical services aligned with the plan of care and infection control standards
- Complete comprehensive patient assessments, including OASIS and medication reconciliation, accurately and timely
- Document all patient visits according to policy and payer requirements and ensure timely synchronization per company standards
Quality
- Perform initial and ongoing nursing evaluations to confirm home health eligibility and medical necessity
- Develop and coordinate patient care plans with physicians within regulatory guidelines
- Communicate effectively with internal staff regarding care issues, scheduling, documentation, and coordination of services
- Maintain timely communication with physicians, patients, and families to ensure quality care and service excellence
Required Qualifications
- Current, unrestricted Registered Nurse (RN) license in the state of Alabama
- Current driver’s license, auto insurance, and access to a dependable vehicle
- Basic Life Support (BLS) certification; ACLS accepted if applicable
Preferred Qualifications
- Self-motivated with strong organizational and time management skills
- Detail-oriented with strong clinical documentation abilities
- Previous home health experience preferred
