Home Health Physical Therapist

Pride Health is hiring a Home Health Physical Therapist to support our client’s medical facility, located in Puyallup, WA 98373. This is a 3 month contract assignment (possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!

Location: Puyallup, WA 98373
Duration: Contract
Pay Range: $50 - $56. on w2/Pay Rate is based on experience and educational qualifications.
Shift: Mon - Fri 8am - 4:30pm.

Travel: Patient homes - Pierce County, WA
Mileage Reimbursement: Yes (between patient visits).

Specific Duties:

• Evaluate patients’ functional status and physical therapy needs in compliance with CMS guidelines.
• Develop and implement individualized treatment plans in collaboration with physicians.
• Deliver evidence-based therapy to relieve pain, restore function, and improve mobility.
• Monitor and document patient progress, reporting changes to physicians and nurse supervisors.
• Educate patients, families, and caregivers on treatment plans, progress, and functional limitations.
• Complete timely and accurate documentation, including evaluations, progress notes, and discharge summaries.
• Travel to patient homes within the assigned service area and nearby locations as needed.
• Participate in care coordination, team meetings, and case conferences.
• Supervise Physical Therapist Assistants (PTAs) and Home Health Aides as applicable.
• Maintain compliance with agency policies, state regulations, and professional standards of practice.
• Participate in continuing education to maintain licensure and clinical competency.

Qualifications:

• Active Physical Therapist license in Washington State (or compact license).
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• Experience in home health or related clinical settings preferred.
