Home Health Nurse

We are seeking an experienced Registered Nurse (RN) – Adult Care for a home care role supporting patients in their own residences. This position involves delivering comprehensive, patient-centered care including assessments, medication administration, post-operative care, and patient education. The ideal candidate will have strong Med/Surg experience, recent clinical exposure, and prior home care field experience.

Key Responsibilities

• Assess, plan, implement, and evaluate individualized patient care plans
• Provide direct bedside care for pre- and post-operative patients in a home setting
• Administer medications and treatments safely and accurately
• Perform clinical procedures including IV therapy, catheter insertion, and wound care
• Monitor vital signs and identify/report changes in patient condition
• Educate patients and families on treatment plans, medications, and post-care instructions
• Collaborate with interdisciplinary teams for coordinated care and discharge planning
• Maintain accurate documentation of patient records, charts, and care updates
• Ensure adherence to infection control practices and clinical protocols
• Manage a daily caseload of approximately 4–8 patient visits

Qualifications

• Associate Degree in Nursing (ASN/ADN) from an accredited program
• Active New York State RN License (with primary source verification)
• AHA BLS certification (ACLS required for select locations)
• Minimum 2 years of recent Med/Surg experience (within the last year)
• Prior experience providing care in patients’ homes (required)
• Strong skills in physical assessment, IV therapy, wound care, and patient education
• Ability to work independently and travel between patient homes
• Experience with EPIC EMR

Important Details:

• 13-week contract position (extensions available)
• Shift is 8 hrs/day Mon–Fri (9:00 AM – 5:00 PM), 40 hrs/week
• Location is Manhattan, NY 10004
• Pay Range for Locals: $74 - $77 per hour
• Pay Range for Travelers: $3,250 - $3,300 per week
