We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide high‑quality nursing care to homebound patients across Shelby, Fayette, and Tipton counties. This role is ideal for an RN who enjoys working independently, educating patients, and delivering hands‑on skilled nursing care in the home setting.
Schedule & Work Details- Schedule: Monday-Friday
- Hours: 8:00 AM - 4:30 PM
- On‑Call / Weekends: Not required
- Mileage Reimbursement: Yes — reimbursed between patient visits, from first to last visit of the day
- Dress Code: Royal blue and/or white scrubs
- Provide skilled nursing care in patients’ homes for homebound individuals
- Perform comprehensive nursing assessments and ongoing patient evaluations
- Educate patients and caregivers on disease processes, treatment plans, and self‑care
- Deliver skilled services including, but not limited to:
- IV care and patient education
- Wound care and monitoring
- Catheter placement, care, and education
- Monitor patient progress and promptly report significant changes to the care team
- Develop and follow individualized plans of care in collaboration with physicians and the interdisciplinary team
- Complete accurate and timely documentation using an electronic medical record (EMR)
- Travel daily between patient homes within the assigned counties
- Active Registered Nurse (RN) license valid in the state of Tennessee
- Experience in home health or home care nursing
- Experience using electronic medical records (EMR)
- Strong clinical assessment and patient‑education skills
- Ability to work independently in a field‑based role
- Experience performing OASIS assessments
- Advanced wound care experience
- Strong time‑management and organizational skills
- Clear, compassionate communication style
- Patient‑focused and detail‑oriented approach to care
