Job Title: Health & Wellness Advisor I / Care Specialist
Location: Hybrid / New York, NY 10004
Shift: Day 5x7-Hour (09:00 - 17:00)
Duration: 2 months probable extension
Pay Range: $24/hr - $27/hr
Required:
1–2 years' experience in care management, healthcare coordination, or outreach preferred
Experience working with high-risk populations (HIV, behavioral health, or chronic conditions) preferred
Education: High School Diploma or GED required
Duties:
Performed telephonic care management including health assessments, care plan development, coordination, and follow-ups to ensure optimal outcomes
Developed and implemented individualized, member-centered care plans in collaboration with healthcare providers, family members, and community resources
Coordinated access to healthcare services for HIV-positive, transgender, and homeless populations
Provided health education using coaching and motivational interviewing techniques to improve medication adherence and overall wellness
Required Skills:
Strong communication and interpersonal skills with ability to build rapport with diverse populations
Knowledge of care management, chronic disease management, and healthcare coordination
Ability to handle high-volume telephonic outreach and multitask effectively
Familiarity with motivational interviewing and patient engagement techniques
Strong documentation, organizational, and compliance skills
