Medical Director – PACE Program (West Springfield, MA)
We are seeking an experienced Medical Director to lead a Program of All-Inclusive Care for the Elderly (PACE) in West Springfield, Massachusetts. This is a full-time leadership opportunity focused on delivering high-quality, community-based care for seniors with complex and chronic medical needs.

Role Highlights:
• Provide clinical and medical oversight for participant care
• Lead and collaborate with an interdisciplinary care team
• Oversee clinical outcomes and Quality Assurance Performance Improvement (QAPI) initiatives
• Develop and maintain medical policies, protocols, and standards
• Supervise Primary Care Providers and Advanced Practice Providers
• Participate in participant assessments, care planning, and family conferences
Requirements:
• Active and unrestricted Massachusetts physician license
• Board Certification in Internal Medicine or Family Medicine (Geriatrics preferred)
• DEA and CDS certification
• Prior supervisory experience required
• Minimum 1 year of experience working with frail or elderly populations
• Strong skills in physical assessment and chronic disease management
• Interest in working within an innovative, community-based care model
