Program: Senior Community Care (SCC)
The Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) will serve as a key clinical provider within the Senior Community Care (SCC) program—an integrated, longitudinal care delivery model supporting high‑risk health plan members residing in facility-based settings. This role focuses on improving clinical outcomes, reducing avoidable hospitalizations, and ensuring high‑quality, patient‑centered care across several skilled nursing and rehabilitation facilities.

Primary Responsibilities- Conduct comprehensive assessments, including physical exams, medication reviews, and chronic disease evaluations.
- Manage both acute and chronic medical and behavioral health conditions for members in skilled nursing and senior living environments.
- Ensure accurate and thorough documentation, including ICD‑10 coding of all relevant conditions.
- Collaborate with interdisciplinary care teams, including physicians, nursing staff, case managers, and facility leadership.
- Lead advanced illness and advance care planning conversations with patients and their families.
- Provide ongoing education, counseling, and support to patients and caregivers to ensure understanding of care plans and treatments.
- Participate in quality initiatives, care coordination, and communication that enhance continuity of care across facilities.
Nurse Practitioner candidates must:
- Hold an active and unrestricted NP license in the state of residence (or be able to obtain by start date).
- Be a graduate of an accredited MSN or DNP program.
- Maintain national certification through:
- AANP or ANCC in one of the following:
- Family Nurse Practitioner
- Adult Nurse Practitioner
- Gerontology Nurse Practitioner
- Adult-Gerontology Acute Care Nurse Practitioner
- AANP or ANCC in one of the following:
Physician Assistant candidates must:
- Hold an active and unrestricted PA license.
- Be a graduate of an accredited PA program.
- Maintain certification through NCCPA.
- Current, active DEA license/prescriptive authority.
- Ability to lift a 30‑pound bag, navigate stairs, and perform comprehensive physical assessments in varied environments.
- Reliable transportation for daily travel between facilities.
- Ability to secure a collaborative practice agreement (if required in NH).
- Full COVID‑19 vaccination, per federal, state, and client requirements (proof required).
- Must pass pre-employment drug screening (UHG is a drug-free workplace).
- 1 year of clinical practice experience (long-term care or senior living preferred).
- Experience managing patients with complex behavioral, social, or medical needs.
- Strong understanding of geriatrics, chronic disease management, and end-of-life discussions.
- Proficiency with EMR systems and electronic documentation.
- Strong communication skills and comfort working in an autonomous, facility-based environment.
- Travel is required between 6 assigned facilities within Rockingham and Strafford counties.
- All travel occurs during scheduled work hours.
Salary : $65 - $70
