Location: Sylva, NC
Job Type: Contract
Contract Length: 13 Week/s
Pay Range: 58/Hour - 62.65/Hour
*Pay offered is based on experience, expertise, credentialing, and education.

• Productivity expectations: 30 points
• On Call/Weekend Requirement: None
• Coverage Areas: Jackson, Swain, Haywood Counties.
• Milage will be reimbursed between visits, from first patient to last patient.

Duties:

• Conduct OT evaluations and develop individualized treatment plans in patients’ homes.

• Provide therapeutic interventions and reassess patient progress to support functional independence.

• Complete discharge planning, documentation, and patient/caregiver education as needed.

Requirements:

• Experience: Home Health experience, Homecare Homebase software experience preferred 
• License(s): Occupational Therapist license - North Carolina
• Certification(s): Basic Life Support - AHA
• Years of experience: 1 year
