Job Summary:

The Clinical Nurse (RN) plays a vital role in ensuring the seamless
coordination of mental health care for clients. This position requires
a licensed Registered Nurse (RN) with strong clinical expertise,
problem-solving abilities, and excellent follow-up skills in a mental
health setting. The ideal candidate will demonstrate patience and
empathy when working with clients, fostering trust and building
supportive relationships.

This role focuses on care coordination, ensuring clients receive
comprehensive, timely, and effective treatment through wraparound
services, including therapy, day rehabilitation, and medication
management. The Clinical Nurse (RN) will work in collaboration with
therapists, psychiatrists, social workers, and primary care providers
to integrate mental health and general healthcare into a cohesive
plan. The position includes daily staff communication, treatment plan
follow-ups, documentation, advocacy, and continuity of care—all
essential for client success.

This is a full-time role with virtual work flexibility, allowing for
remote coordination while maintaining in-person support when
necessary.

Care Coordination in Mental Health

Mental health care coordination is the structured effort to ensure
clients receive high-quality, holistic care tailored to their unique
needs. It involves streamlining communication among healthcare
providers, minimizing treatment gaps, and ensuring smooth transitions
across services.

Key Components of Mental Health Care Coordination:

Interdisciplinary Collaboration: Align treatment plans across
wraparound services like therapy, day rehabilitation and medication
Management.

Primary Care Integration: Ensure clients maintain access to a primary
care provider for medical oversight alongside mental health support.

Treatment Plan Follow-Up: Monitor client progress, evaluate
effectiveness, and collaborate with providers.

Resource Navigation: Assist clients in obtaining community services,
financial aid, and additional healthcare resources.

Continuity of Care: Support transitions between different care
settings, ensuring consistent follow-ups and coordination.

Advocacy & Education: Empower clients and families with information
about mental health resources, helping them engage in their own care.

Key Responsibilities:

Care Coordination: Develop and oversee personalized treatment plans
integrating wraparound services such as therapy, day rehabilitation,
and medication management.

Virtual & In-Person Support: Effectively coordinate care remotely
while engaging in direct client interactions as needed.

Treatment Plan Monitoring & Follow-Up: Assess client progress, ensure
adherence to care plans and adjust strategies when necessary.

Daily Staff Collaboration: Maintain strong communication with
healthcare teams to align care strategies and address client needs.

End-of-Day Reporting: Provide structured updates to relevant
departments beyond case notes.

Client Engagement & Trust Building: Foster meaningful relationships
with clients to enhance treatment outcomes and provide emotional
support.

Clinical Documentation: Accurately record patient progress in
electronic health records (EHR) systems while ensuring compliance with
healthcare regulations.

Problem-Solving & Follow-Up: Address challenges in client care,
proactively identifying solutions and ensuring timely responses.

Resource Navigation: Connect clients with essential wraparound support
services, including housing assistance and employment programs.

Transition Management: Guide clients through changes in care settings
while maintaining continuity of care.

Advocacy & Education: Educate clients and families about mental health
resources, helping them navigate the healthcare system effectively.

Qualifications & Skills:

Active RN license in [State/Region].

Expert knowledge of mental health services, care coordination, and
treatment plans.

Exceptional communication and interpersonal skills for team
collaboration and client engagement.

Strong problem-solving and follow-up skills to ensure effective case management.

Proficiency in electronic health records (EHR) systems for accurate
and timely documentation.

Ability to work with patience and empathy in high-stress mental health
environments.

Comfortable working in a virtual setting with the ability to
coordinate care remotely when needed.

Preferred Experience:

Mental health nursing experience, particularly in care coordination.

Familiarity with community mental health programs and wraparound services.

Case management experience involving interdisciplinary healthcare teams.

Salary : $50 - $60
