What are the responsibilities and job description for the Direct Service Worker position at Preventive Measures Programs, Inc?
REPORTS TO: The DSW reports directly to the assigned DSW Supervisor.
MINIMUM QUALIFICATIONS:
Must be at least 18 years of age.
Education and Training: A high school diploma or GED.
Experience and Qualifications: Requires knowledge of general care and training of the
elderly and physically and mentally disabled people. Healthcare experience is preferred, but
not required. Must have a valid Louisiana driver’s license with a good driving record.
Must submit to and pass criminal background check.
Must possess the ability to read and understand treatment plans, prescription medication and directions for the care of others.
Must be able to work flexible shifts, weekends, and holidays.
Must demonstrate initiative, empathy, consistency, and motivation while working with clients.
Requires the ability to deal with the public in a pleasant, efficient, and professional manner. Must be capable of working with a variety of people.
Have good decision-making skills.
Ability to work independently with minimum supervision.
Ability to communicate orally and in writing.
Must demonstrate appropriateness in dress and behavior.
FUNCTIONS/OVERVIEW:
The DSW is responsible for the primary care of the clients who meet the specific eligibility criteria. As such, the DSW will provide individualized planning and coordination for services identified by each client’s Interdisciplinary Team, while working in the client’s home.
PRINCIPLE ACTIVITIES/RESPONSIBILITES:
o Assist with personal hygiene, dressing, bathing, grooming, toileting, ambulation, transferring, light housekeeping, shopping, laundry, medication reminders and other assigned tasks as it relates to the client’s plan of care (POC) or comprehensive plan of care (CPOC).
o Provides services outlined in the Service Plan to prevent out of home placement.
o Advises DSW Supervisor of all problems, critical incidents, tardiness, and absences.
o Provides recreational activities.
o Prepare and serve food.
o Reports violation and abuse allegations to parents and/or DSW Supervisor.
o Maintains client confidentiality.
o Complies with rules/regulations according to agency guidelines, state and federal licensing and certification requirements).
o Practices preventive safety and health techniques.
o Maintains daily time for involvement with clients as scheduled.
o Maintains and timely submit accurate documentation of timesheet, logs and progress notes.
Job Type: Part-time
Pay: $8.00 - $10.00 per hour
