JOB DESCRIPTION
Direct Service Worker (DSW) Supervisor
MINIMUM QUALIFICATIONS:
Education and Training: A high school diploma or GED.
Experience and Qualifications: Must have a minimum of two (2) years’ work experience in working with the elderly, individuals with disabilities, MR/DD or similar type clients. Previous supervisory experience preferred.
Requires the ability to deal with the public in a pleasant, efficient, and professional manner.
Must be flexible and capable of working with a variety of people.
Must have good decision-making skills.
Ability to work independently with minimum supervision.
Ability to communicate orally and in writing.
FUNCTIONS/OVERVIEW:
The DSW Supervisor is responsible for the supervision and coordination of the day-to-day activities with staff providing direct client care in the client’s home.
PRINCIPLE ACTIVITIES/RESPONSIBILITES:
- Makes at least two monthly supervisory visits to the clients’ home.
- Monitor and observe DSW work ethics and performance when providing care in the home.
- Responsible for coordinating schedules for staff and clients. (Monitor daily)
- Collects, review for accuracy and file weekly logs and progress notes.
- Reviews all documentation for accuracy, appropriateness, and completeness.
- Collects data about the health status of the client.
- Monitor client service authorization and check benefit eligibility monthly.
- Serves as a member of the ID team and assist in establishing the plan of care for client.
- Documents progress toward goal achievements.
- Responsible for collecting employees’ documents required for their individual personnel folder.
- Responsible for collecting, reviewing, and documenting the receipt of logs, progress notes and timesheets as directed by the Administrative/Director.
- Complete employee performance evaluations (30 days, 90 days and annual).
Job Type: Full-time
Pay: $12.00 - $20.00 per hour
Expected hours: 40 per week
