Requirements
- Active LPN license and BLS certification
- Minimum of 18 months experience in SNF or LTC rehab settings
- Prior skills in trach management, Foley care, wound care, dressing changes, IV management, and tube feedings
- Experience with EMR systems (EPIC preferred) and equipment such as Alaris IV pumps and Omnicell medication dispensing
- Ability to float between facilities in the care region as needed
- Must be able to work the night shift (2300–0700), with every other weekend and 75% of holidays
- Provide skilled nursing care to patients in a rehab and long-term setting
- Manage complex patient needs, including IV therapy, wound care, and trach management
- Collaborate with interdisciplinary teams including therapies, case management, and admin staff
- Document patient care accurately using EMR systems
- Float within designated facilities and support team efforts as required
- Adhere to facility protocols and safety standards
- Steady, predictable 8-hour night shifts with flexible scheduling options
- Paid travel time when covering multiple facilities within the region
