POSITION SUMMARY

The Resident Care Specialist (RCS) supports all aspects of health services systems and resident care and drives comprehensive health services results under the direction of the Senior Regional Support Nurse and Director of Clinical Services in coordination with assigned community leadership. The Resident Care Specialist ensures resident care needs are met by supporting the coordination of resident care, supporting health services systems, and providing training and support to health services team members. The Resident Care Specialist ensures the highest level of care for residents in accordance with regulatory and Prestige standards.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Partners with Regional Support Nurses (RSN) to provide health service support as assigned including survey preparation, auditing, implementing systems, supporting plans of correction, providing onsite support and training, and monitoring for compliance.
  2. Partners with assigned community to complete a comprehensive review of medication services including staff training, medication coordination, organization of medication room, and implementation medication systems. Identifies opportunities for improvement and implements plan of corrections.
  3. Conducts audits of health services systems and assesses community compliance with health services policies and procedures and state and federal regulations. Reports findings to community and regional leadership. Partners with community and RSN with the development and completion of plans of correction based on audit results and monitors progress towards meeting expectations.
  4. Performs QAPI and survey preparation audits, reports on audit findings, and leads efforts to create and complete plans of corrections. Partners with RSN and RDO to ensure community completes plan and is survey ready.
  5.  Provides resident care and completes evaluations as assigned by health services leadership.
  6.  Develops, implements, maintains, and facilitates education programs and trainings on health services including medication administration, caregiving and activities of daily living (ADL), electronic health records and health services documentation, care planning, and integration of care.
  7. Coordinates and assists with training new health services team members as assigned.
  8.  Provides mentorship to health services team members. Serves as subject matter expert on health services systems and policies and procedures.
  9. Acts as a Central Support resource for Resident Care Coordinators (RCC). Provides training and support to RCCs.
  10.  Makes routine site visits to Prestige communities to provide health services support. Performs site visits in a professional, respectful, and productive manner. Provides site visit reports to Executive Director and appropriate regional team members.
  11. Problem solves with community leadership and collaborates with regional team members to remove barriers to drive comprehensive health services results
  12.  Attends and participates in regional and Central Support meetings as required. Reports on assignments and makes recommendations for improvements.
  13. Provides on-call support and coverage for vacancies in the Resident Care Coordinator and health services leadership roles as needed. Works as a Medication Technician and/or Personal Care Attendant as needed to cover shifts in a staffing emergency.
  14.  Maintains professional knowledge of industry trends and best practices through continuing professional education, maintaining personal networks, and participating in professional organizations.
  15. Performs other duties as assigned.

QUALIFICATIONS AND EXPERIENCE

  • Minimum 3 years of related experience in senior living with strong knowledge of health services systems. Resident Care Coordinator experience strongly preferred. Must meet caregiving and medication technicians requirements by state or ability to obtain upon hire. Strong knowledge of health services systems and electronic health records required. Leadership and training experience preferred. Strong interpersonal and communication skills with exceptional customer service background.

EDUCATION

  • High school diploma, GED, or equivalent knowledge and skills obtained through a combination of education, training, and experience. Associate degree or bachelor’s strongly preferred.

SUPERVISORY RESPONSIBILITIES

  • This position has indirect supervisory responsibilities.

LANGUAGE SKILLS

  • Ability to communicate effectively, in English, both verbally and in writing, with personnel at all levels of the organization, residents and their families, and members of the community.

MATHEMATICAL SKILLS

  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to maintain, understand, and review all reports and records.

REASONING ABILITY

  • Ability to accurately evaluate, troubleshoot, and problem-solve to meet the needs of the team.
  • Develop training for team member needs and evaluate results.

WORK ENVIRONMENT

  • Primarily works in an assisted living or memory care community in office spaces, medication rooms, and resident areas. Travels to Prestige communities up to 100% of the time

PTO:
AL/PCI Salary: Accrue PTO at rate of 0.0615/hour during their first year/16 days.
PCI Hourly: Full-time and Part-Time team members (over 20 hours/week) accrue PTO at rate of 0.0423/hour during their first year.
