Duties
- Conduct comprehensive patient assessments and diagnostic evaluations to determine mental health needs and develop personalized treatment plans.
- Manage case records with precision using Electronic Medical Records (EMR) systems, ensuring HIPAA compliance and accurate medical documentation.
- Coordinate care with multidisciplinary teams including medical professionals, educators, and community agencies to facilitate holistic support.
- Implement specialized interventions for populations with developmental disabilities, autism spectrum disorder (ASD), PTSD, addiction issues, geriatrics, or Alzheimer’s care.
- Provide home visits and community outreach services to support patients in their natural environments.
- Conduct intake assessments for new clients, ensuring thorough understanding of individual needs and goals.
- Offer crisis management and emergency response during acute mental health episodes or behavioral crises.
