Position Summary

The Permanency Specialist works in a collaborative manner to build a partnering relationship with families and others in the home and community to assure that the needs of designated children and their families are respectfully and adequately addressed. In each case, the Permanency Specialist will assure that services to children and families are individualized, strengths and needs based, culturally sensitive, responsive, and goal driven. This position requires travel and flexibility to work a nontraditional schedule in a high-demand work situation.

Essential Responsibilities

  1. Gathers and understands current and past social, medical, educational, psychological, and work histories.
  2. Uses diagnostic tools to provide comprehensive information for a service plan.
  3. Assesses risk factors related to safety and well-being of all family members.
  4. Provides clinical direction that meets the needs of the child and family
  5. Plans and holds discussions with the County or International agency for the purpose of creating service plans/ISPs.
  6. Works with family members to develop ownership for the service plan/ISP.
  7. Writes and utilizes specific measurable goals, objectives, and time frames for each child and family.
  8. Develops crisis plan for each child and family.
  9. Completes written documents and services contracted through private, County or International agency referrals.
  10. Maintains flexibility and creativity in working a non-traditional schedule to accommodate the needs of children and families.

Qualifications

  1. Education/Credentials/Licensure:
    1. Requires a bachelor’s degree in human services or related field.
    2. Requires a valid state driver’s license and auto insurance.
  2. Clearances Required by Applicable Regulations:
    1. State Police
    2. FBI
    3. Child Abuse Clearance
    4. Mandated Reporter-Recognizing and Reporting Child Abuse training certificate and/or statement

Working Conditions 

  1. Physical Demands: 
    1. This position requires operating a keypad device 25% of the day and electronic/office equipment 25% of the day. Travel is required approximately 5 times per month and works in weather conditions about 25% of the day. Travel includes transporting students to and from school, school-based appointments, and/or field trips. Fingers, thumbs, hands, wrists, elbows, legs and feet are all routinely used throughout the day. The lower and upper torso is used 75% of the day. Employees must meet minimum requirements for hearing, speech, and vision. Walking is required 50% of the day. Standing occurs 25% of the day. Sitting happens 25% of the day. Reaching above the shoulder as well as bending and twisting are performed 25% of the day. This function routinely lifts 15 pounds, 4 times per day. The maximum lift is 50 pounds, 1 time per day. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
  2. Mental Demands:
    1. Continually handles distractions that interfere.
    2. Frequently perform multiple tasks, make quick decisions, and concentrate.
    3. Occasionally handles a diversity of problems.
  3. Environmental Factors: 
    1. Indoors/Outdoors: school, home, community.
  4. Working Hours:
    1. Non-traditional hours as determined by service needs.
