- Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, memory care, and behavioral health conditions.
- Manage wound care, tube feeding, and sterile procedures using aseptic techniques to ensure optimal healing and safety.
- Educate patients and their families on disease management, medication administration, nutrition, and self-care strategies to promote health literacy.
- Document all patient interactions accurately within electronic health record (EHR) systems like Epic or Cerner while maintaining compliance with HIPAA standards.
- Collaborate with interdisciplinary teams including physicians, case managers, and therapists to coordinate seamless home-based care delivery.
Experience
- Proven LVN experience in diverse settings such as home health, long-term care facilities, assisted living, or hospitals.
- Critical care experience including ICU or emergency room work is highly valued for managing acute situations effectively.
- Familiarity with medical documentation systems such as Epic, eClinicalWorks, or Athenahealth enhances workflow efficiency.
- Hands-on experience with IV infusion therapy, medication administration (including injections), phlebotomy, and catheterization procedures.
- Knowledge of ICD coding (ICD-9/10), CPT coding, and diagnostic evaluation techniques to support accurate documentation and billing.
- Prior work with individuals with disabilities or developmental delays demonstrates your ability to deliver compassionate specialized care.
