Starting Pay: $21.00/hour - $26.00/hour; Depending on experience

The Service Coordinator/Social Worker is responsible for establishing ties to service providers and agencies in the community, referring residents to such providers and monitoring the provision of these services. The Service Coordinator is responsible for the monitoring of services that address the social determinants of health and bridge gaps between communities, individuals and providers.

- Have a bachelor's degree (Social Work, Gerontology or a related field a plus!) OR four years related experience.
- Have a valid Pennsylvania driver’s license and access to a registered and insured vehicle for frequent and regular travel between communities.
- Able to effectively interact with residents and team members.
Salary : $21 - $26
