Role Description
This is a full-time, on-site role for a Post Acute Cardiac Advanced Practice Provider (APP) located in Jacksonville, FL and surrounding area. The APP will collaborate with physicians and healthcare teams to provide patient-centered cardiac care. Day-to-day responsibilities include conducting patient assessments, diagnosing and managing cardiovascular conditions, creating and implementing care plans, performing follow-up evaluations, and maintaining accurate medical records. The APP will also educate patients and staff on cardiac care practices and ensure adherence to evidence-based clinical guidelines in a post-acute care setting.
Qualifications
- Proficiency in patient evaluations, diagnosis, and management of cardiovascular conditions
- Experience in developing and implementing personalized care plans and assisting with post-acute care needs
- Knowledge of evidence-based medical practices, diagnostics, and treatment protocols in cardiology
- Strong communication and patient education skills to foster collaborative care environments
- Active Nurse Practitioner (NP) license in the state of Florida
- Cardiology specialization or previous experience in post-acute care settings preferred
- Ability to work effectively in an interdisciplinary healthcare team
- Current ACLS/BLS certifications required
