Position Responsibilities
- Evaluate and treat residents in a skilled nursing facility setting
- Develop individualized treatment plans
- Collaborate with physicians, nursing staff, families, and interdisciplinary team members
- Complete timely and compliant documentation
- Help patients achieve their highest level of functional independence
