Position Summary

Provides comprehensive primary care within Nuvia Medical’s value-based care model.

Works with care coordination and quality teams to improve outcomes and close gaps in care.

Essential Duties & Responsibilities

• Deliver full-spectrum outpatient primary care.
• Manage a defined patient panel in a value-based care environment.
• Close gaps in care with quality and care coordination teams.
• Ensure compliance with HEDIS quality measures.
• Promote appropriate utilization of healthcare services.
• Develop individualized care plans for complex patients.
• Participate in population health initiatives.
• Review clinical dashboards and implement improvements.
• Educate patients and families on disease prevention.
• Document encounters accurately in the EHR.
• Participate in interdisciplinary meetings and peer review.
• Support Nuvia’s culture of excellence and accountability.

Minimum Qualifications

• MD or DO from an accredited institution.
• Board Certified or Board Eligible in Family Medicine.
• Active Florida medical license.
• Active DEA registration.
• Experience in outpatient primary care preferred.
• Knowledge of value-based care and population health.
• Strong communication and teamwork skills.
