Overview
Premier Medical Associates is seeking an experienced, patient-focused Advanced Practice Registered Nurse (APRN) to join our Hospitalist team in The Villages. This role is primarily responsible for caring for hospitalized adult patients, working closely with our physicians, nurses, case management, and ancillary staff to deliver high-quality, coordinated care.
Key Responsibilities
Patient Care & Rounding
- Perform daily hospital rounds on assigned patients, including admission, follow-up, and discharge visits.
- Conduct comprehensive histories and physicals, review diagnostics, and develop evidence-based treatment plans.
- Manage acute and chronic medical conditions in collaboration with supervising physicians.
- Write orders for medications, labs, imaging, and therapies within scope and hospital bylaws.
- Provide timely documentation in the EMR, including admission notes, progress notes, consult notes, and discharge summaries.
Collaboration & Communication
- Collaborate with attending physicians, specialists, nursing staff, case managers, and other team members to coordinate patient care.
- Communicate clearly with patients and families about diagnoses, treatment plans, and discharge instructions.
- Participate in multidisciplinary rounds, care conferences, and quality improvement initiatives as requested.
Care Coordination & Quality
- Support transitions of care between the hospital and outpatient providers to reduce readmissions.
- Ensure care is aligned with hospital protocols, value-based care initiatives, and evidence-based guidelines.
- Monitor clinical status, respond to changes in patient condition, and escalate to the attending physician as needed.
- Assist with medication reconciliation at admission and discharge.
Compliance & Professionalism
- Adhere to hospital policies, state and federal regulations, and APRN scope-of-practice regulations.
- Maintain accurate, timely, and compliant documentation in the electronic medical record.
- Participate in ongoing education, required hospital trainings, and performance/quality reviews.
Qualifications
- Current, unrestricted Florida APRN license.
- National certification as a Nurse Practitioner (AGACNP, AGNP, FNP, or similar accepted based on hospital bylaws).
- Valid DEA registration (or eligibility to obtain).
- Minimum 1–2 years of inpatient or hospitalist experience preferred; strong new graduates with applicable clinical rotations may be considered.
- Current BLS and ACLS certifications.
- Strong clinical assessment, critical thinking, and problem-solving skills.
- Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced hospital environment.
- Comfort using electronic medical records (EMR) and hospital order-entry systems.
Schedule & Work Environment
- Full-time position based at The Villages area hospital.
- Schedule may include day, evening, weekend, and holiday shifts, with rotation based on service needs.
- Primarily inpatient rounding with potential cross-coverage of step-down and observation units as needed.
