Responsibilities

• Assist patients with activities of daily living such as bathing, dressing, feeding, and mobility.
• Provide specialized care for individuals with dementia, Alzheimer's disease, memory impairments, developmental disabilities, or behavioral health needs
• Assist with transfers and mobility using appropriate equipment while adhering to safety protocols for patient and caregiver safety
• Maintain compliance with HIPAA regulations by safeguarding patient information and ensuring accurate medical documentation

Qualifications

• Valid certification as a CNA (Certified Nursing Assistant) or PCA (Patient Care Assistant) with current licensure where applicable
• Experience working in various healthcare settings for home care environments
• Ability to perform basic life support (BLS) certification and respond effectively
• Prior experience with specialized populations such as pediatrics, geriatrics, behavioral health patients or those requiring hospice or memory care services is highly desirable
• Strong communication skills combined with a compassionate approach to patient service and caregiving
