About the Role
We are seeking a motivated and organized Community Outreach Coordinator to support outreach activities, facility engagement, and community partnerships across Maryland.
This is a field-based contract role involving in-person visits to healthcare facilities and community partners, communication with referral sources, and accurate tracking of outreach activity using internal systems.
The ideal candidate is dependable, detail-oriented, and comfortable working independently in the field while managing communication, follow-ups, and documentation.

Schedule & Location
- Field-based across Maryland (Anne Arundel, Howard, Prince George’s, Montgomery, and surrounding counties)
- Flexible schedule based on outreach needs
- Daily travel required (not remote or office-based)
- Schedule and hours are subject to change based on organizational needs.
Key Responsibilities
- Conduct in-person visits to healthcare facilities, hospitals, and community partner locations
- Build and maintain professional relationships with referral sources and partner organizations
- Perform inbound and outbound phone communication for outreach and follow-ups
- Respond to emails and maintain professional communication with internal and external contacts
- Attend community events, networking opportunities, and outreach activities as assigned
- Accurately document outreach activity and maintain records using internal tracking systems
- Use internal systems to log communication, track outreach efforts, and ensure consistent follow-up
- Support community engagement efforts across assigned service areas
Requirements
- High school diploma or GED required
- CPR/First Aid Certification (American Heart Association or American Red Cross – required and current)
- Minimum 2 year of experience in customer service, healthcare support, caregiving, or community outreach preferred
- Strong computer literacy and comfort using digital systems (Microsoft Word, Excel, email, data entry, and basic Customer Relationship Management (CRM)/tracking systems)
- Ability to quickly learn and navigate internal systems for documentation and outreach tracking
- Strong organizational skills, attention to detail, and consistent follow-up habits
- Excellent written and verbal communication skills
- Valid driver’s license and reliable transportation required
- Must be 18 years or older
- Must be able to successfully pass Maryland State and Federal background checks
- Must pass drug screening, medical screening, and TB test
- Comfortable working in a field-based role with daily travel across assigned regions
