- Coordinate the development and implementation of the resident assessment process
- Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
- Ensure accurate and timely completion of MDS assessments
- Collaborate with healthcare professionals to gather and document resident data
- Maintain confidentiality and adhere to HIPAA guidelines
- Participate in interdisciplinary team meetings to discuss resident care plans
- Develop and maintain a thorough understanding of MDS regulations and guidelines
- Current RN or LPN license in the state of Pennsylvania
- Prior experience in MDS required
- Strong organizational and communication skills
- Ability to work effectively with residents, families, and healthcare professionals
- Proficiency in electronic medical records and MDS software
