About the Licensed Practical Nurse LPN Role:
As a Licensed Practical Nurse (LPN) Per Diem at Pleasant Acres Nursing & Rehabilitation Center, you will:
- Provide direct patient care to our residents, including vital sign assessments, medication administration, and wound care
- Collaborate with our interdisciplinary team to develop and implement individualized care plans
- Monitor and report changes in resident condition to the charge nurse and/or physician
- Participate in Quality Improvement initiatives to ensure the highest standards of care
- Demonstrate a strong commitment to patient safety and falls prevention
- Active and unrestricted LPN license in the state of Pennsylvania
- Prior experience preferred; new graduates welcome
- Passion for delivering exceptional patient care and customer service
- Ability to work a flexible per diem schedule, including days, evenings, and weekends
- Strong communication and collaboration skills
- Proficiency in electronic medical records and other software applications
