About the position
Responsibilities
• Provide home visits to 25-30 pregnant and parenting mothers using the Nurse Family Partnership model at 85% productivity or better.
• Document work schedule in Outlook including client visits and other related meetings.
• Complete documentation within 3 working days.
• Conduct screenings, tools, and family plans on time 95% of the time.
• Attend weekly staff meetings and assist with orientation of new staff.
• Promote a seamless continuum of care between all aspects of care and community resources.
• Collaborate with the Home Visit Coordinator to identify learning needs and formulate self-development goals.
• Create a positive work environment that promotes productivity.
• Demonstrate willingness to accept and efficiently process new families.
Requirements
• Bachelor's degree in Nursing or an Associate degree in Nursing (ADN) with a signed MOU to obtain a BSN within five years of hire.
• Licensed Registered Nurse in Ohio.
• 1 - 3 years of job-related experience, preferably in home visiting, OB/Gyn, Pediatrics, Adolescents, or Mental Health.
• At least 1 year of experience working with pregnant women, infants, and children is preferred.
• Must undergo a criminal background check and a background check with the Ohio central registry on child abuse and neglect.
Nice-to-haves
• Experience in a variety of healthcare settings such as OB/Gyn, Pediatrics, or Mental Health.
• Effective verbal, written, and interpersonal communication skills.
• Organized and self-directed.
