Job Overview – Value-Based Care Internal Medicine Physician
We are seeking a compassionate, patient-centered Internal Medicine Physician to join our value-based care practice. The ideal candidate will be experienced in managing complex adult patients across the continuum of care and motivated by outcomes, not just visit volume. You will work in a highly collaborative, team-based environment focused on prevention, chronic disease management, and closing care gaps to improve quality and reduce avoidable hospital utilization.
This role includes an incentive structure with bonus compensation tied to clearly defined metrics, such as quality scores, patient experience, access, documentation accuracy, and overall panel performance in a value-based model.
Key Responsibilities
- Provide comprehensive primary and preventive care for adult patients, including evaluation, diagnosis, and management of acute and chronic conditions in an outpatient, value-based setting.
- Proactively manage a patient panel with particular attention to high-risk, multi-morbid patients, including appropriate referrals, care coordination, and follow-up.
- Utilize evidence-based guidelines and clinical pathways to optimize chronic disease control (e.g., diabetes, hypertension, COPD, heart failure).
- Participate in population health initiatives, including:
- Closing HEDIS and quality care gaps
- Medication reconciliation and adherence
- Annual wellness visits and risk assessments
- Preventive screenings and immunizations
- Coordinate care with nurse practitioners/physician assistants, care managers, pharmacists, and specialists to ensure a seamless patient experience.
- Use the electronic health record (EHR) effectively for accurate documentation, coding, and risk adjustment in a value-based environment.
- Educate patients and families on disease prevention, self-management, and lifestyle modifications.
- Participate in quality improvement activities, clinical meetings, and case reviews to continually enhance care processes and outcomes.
- Contribute to a culture of accountability, teamwork, and patient-first service.
Bonus & Performance Metrics
In addition to base salary, physicians are eligible for performance-based bonuses tied to transparent metrics that may include:
- Quality and clinical outcomes (e.g., chronic disease control, preventive care completion)
- Patient experience and satisfaction scores
- Access and panel management (e.g., timely follow-up, after-hours access, panel engagement)
- Documentation and coding completeness/accuracy in support of value-based contracts
- Appropriate utilization and stewardship of healthcare resources

Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Board certified or board eligible in Internal Medicine.
- Active, unrestricted medical license (or eligibility) in the state of practice.
- Prior experience in a value-based, population health, or managed care environment is highly preferred, but not required.
- Strong clinical skills in adult primary care and chronic disease management.
- Comfortable working in a team-based model with advanced practice providers, care managers, and support staff.
- Excellent communication skills and a demonstrated commitment to compassionate, patient-centered care.
- Willingness to embrace data-driven practice, quality metrics, and continuous improvement.
