Premier Genesee Center for Nursing & Rehab is seeking a compassionate and experienced Social Worker to join our dedicated team. In this vital role, you will support residents and their families by addressing social, emotional, and financial needs, ensuring the highest quality of care throughout their stay.
Social Worker Job Description:
Discharge planning (DC planning) and coordination of post-acute care.
Utilization review and insurance updates to ensure proper coverage and care continuity.
Conducting biopsychosocial assessments to evaluate residents’ needs.
Arranging services and connecting residents with community resources.
Supporting residents and families with social, emotional, and financial concerns.
Collaborating with medical staff and interdisciplinary teams to meet residents’ needs.
Social Worker Key Responsibilities:
Develop, implement, and evaluate social service programs for residents.
Lead discharge planning, resident assessments, and individualized care plan development.
Conduct interviews with residents and families to gather social histories and provide support.
Educate residents and families on Medicare, Medicaid, and financial assistance programs.
Maintain thorough, accurate, and timely progress notes.
Support quality assurance efforts and assist during surveys and inspections.
Actively participate in committee meetings (Infection Control, Policy Advisory, Quality Assessment).
Ensure residents’ end-of-life care decisions (MOLST/POLST) are honored and documented.
Social Worker Requirements:
CSW (Certified Social Worker) or LSW (Licensed Social Worker) required.
1 year of experience in social services, with nursing home experience preferred.
Strong communication, interpersonal, and problem-solving skills.
Ability to work collaboratively within a team environment.
