As we expand services, we are seeking an experienced and ambitious Waiver Case Manager, someone ready to not only manage a caseload but also help build the foundation of a growing team. This position is designed for a strong candidate who may step into a Lead or Supervisory role in the near future.
Position Summary
In this role, you will coordinate person-centered waiver services that help individuals live as independently and safely as possible in the community. You'll work with diverse populations and manage the full spectrum of support—from intake and care planning to service coordination and outcome monitoring. Beyond day-to-day case management, you'll have the chance to collaborate on systems development, coach peers, and take on leadership responsibilities as our team grows.
Note: The first 90 days of this position will be in-person onboarding at our main office. A hybrid schedule may be approved earlier for candidates with demonstrated familiarity with Ramsey County waiver case management protocols
Key Responsibilities
- Build strong relationships with individuals served and their support networks.
- Complete assessments, develop and implement service plans using person-centered principles.
- Maintain ongoing communication with service providers and interdisciplinary teams.
- Conduct monthly contacts and biannual in-person visits per waiver requirements.
- Coordinate and monitor the delivery of waiver and non-waiver services (CADI, BI, EW, DD).
- Maintain accurate documentation, case notes, and service authorizations to meet DHS and county standards.
- Assist with appeals, provider issues, and emergency/crisis response planning.
- Collaborate with leadership to shape internal processes and support team quality standards.
- Support onboarding and mentoring of new staff as the team expands.
Required Qualifications
- Bachelor’s degree in social work, psychology, sociology, nursing, or a related field.
- At least 1 year of experience providing waiver case management services (CADI, BI, EW, or DD).
- Demonstrated use of person-centered and culturally responsive practices.
- Strong verbal and written communication skills.
- Proficient with EHRs, MS Office, and case documentation platforms.
- Ability to travel to client homes and community locations.
- Driver’s license insured reliable vehicle, and successful completion of DHS background study.
Applicants with slightly less than 1 year of experience may be considered if they demonstrate exceptional readiness for leadership and strong knowledge of waiver programs.
Preferred Qualifications
- Experience managing waiver caseloads in Ramsey County strongly preferred
- One year of experience with people with developmental disabilities or related coursework.
- Familiarity with MA, Medicare, and Minnesota’s HCBS waiver system.
- Demonstrated leadership potential and interest in growing into a leadership or supervisory role.
- Experience onboarding peers or participating in internal quality improvement.
