We are seeking an experienced HCS Case Manager to join our team—someone who brings confidence, professionalism and coordinating high-quality, person-centered services.

You will provide comprehensive case management services to the individuals supported in our Foster Family Community. This is a leadership role with real influence over outcomes and quality of care.
Key Responsibilities
- Ensure all services are delivered in full compliance with HCS Program Provider Survey and Certification Standards and billing guidelines
- Coordinate and monitor residential, vocational, and community-based services in alignment with each individual’s Individual Plan (IP) and IPC
- Develop and implement individualized strategies that promote independence, skill development, and reduced support needs over time
- Supervise, coach, and support families to maintain high standards of care
- Provide ongoing encouragement, guidance, and performance support
- Serve as a strong advocate for individuals, ensuring dignity, choice, and self-direction
Qualifications
- High school diploma or GED with a minimum of 2 years of experience in social services, behavioral health, human services, or a related field
- Bachelor’s degree in Human Services or related field preferred
- Prior experience in HCS, I/DD services, or a closely related field strongly preferred
