Are you a Registered Nurse who is passionate about leading teams, strengthening systems, and delivering exceptional care to older adults? At Prelude Homes & Services, we are seeking an RN Support Nurse to provide clinical services at our Woodbury campus, a 90-bed Assisted Living and Memory Care community.
Position Summary:
The RN Support Nurse works in partnership with and reports directly to the Director of Clinical Services, serving as the on-site clinical team to ensure residents receive high-quality, individualized care while supporting and guiding the care team. This role is ideal for an RN who enjoys a balance of leadership, hands-on clinical oversight, and relationship-building with residents and families.
What You’ll Do:
- Complete resident assessments, develop care plans, and ensure ongoing evaluation of care needs
- Provide supervision, coaching, and support to direct care staff
- Provide day-to-day clinical needs
- Partner with families, providers, and interdisciplinary teams to ensure coordinated care
- Ensure compliance with Minnesota assisted living regulations and company policies
- Oversee medication management systems and clinical documentation
- Monitor changes in condition and proactively respond to resident needs
- Drive quality improvement, infection control, and best practices in resident care
What You’ll Bring:
- Active Registered Nurse (RN) license in Minnesota
- Experience in assisted living, memory care, or long-term care preferred
- Strong leadership, communication, and organizational skills
- Ability to manage multiple priorities across a dynamic environment
- A passion for serving seniors and supporting team development
